Diabetes Disease Management

What is it?
The Diabetes Disease Management Program is a program designed for diabetic members to provide them with information about diabetes and their lifestyle. The information includes topics such as blood sugars, eating right, medications, exercise, recommended tests and more. These mailings occur on a quarterly basis. It does not replace the direction and guidance of your doctor; however it compliments and reinforces the information received in the doctor’s office. The goal of the program is to provide tools to educate our members on promoting improved health through better prevention, detection, treatment and education.

Who is eligible?

All Sanford Health Plan, Sanford Health Plan of Minnesota members who have a diagnosis of diabetes.

How do I join?

To participate in the program, print and complete the Health Risk Assessment Form, and mail it to:
