Evaluation Process

Please call 605-328-9290 to schedule an appointment. Be prepared to provide the following information: name, address, birth date, social security number and name of referring physician. The Transplant Center, where the evaluation will take place, is located in Medical Building 2 (MB2) on the Sanford Campus. Parking is conveniently located in parking ramps near MB2 or surface parking directly in front of the building.  Free Valet Parking is also available in front of Medical Building 3 (MB3). For patients who must travel quite a distance to Sioux Falls, there may be times or circumstances under which some testing may be done in your local areas.
